Important Information

This application requires approval by our Assisting Pastor and Event Coordinator. After reviewing your proposed offsite event and transportation request, we will contact you to let you know if it’s approved.

All drivers need to sign our Church Bus or Van Use Policy ASAP. Please pick up a copy at the front desk.

Every adult or minor riding the bus or van will need to sign a “Transportation Release and Waiver Liability Form for Adults” or a “Release of Liability and Medical Release – for Minors” before going on the bus or van. Please be sure to have copies available for each person riding the bus or van. Copies are available at the church.
